AFFIDAVIT OF CORRECTION
I, ELIAS BAGI RAMIREZ, of legal age, married, Filipino and a resident of 875
Zapote St., Barangay Dadiangas East, General Santos City, after having been duly sworn to in
accordance with law, do hereby depose and say;

That I am the same ELIAS BAGI RAMIREZ who was born to spouses SOLEDAD
DALMIM2ON BAGI and PORCARIO RAMIREZ on December 19, 1969 at Jose Abad
Santos, Davao Del Sur

That in my School, BIR, Philhealth, Comelec record and Certificate of Baptism, my

date of birth was erroneously indicated thereat as DECEMBER 16, 1970 instead of
DECEMBER 16, 1969;

That the name ELIAS BAGI RAMIREZ with date of DECEMBER 16, 1970as
appearing in my School, BIR, Philhealth, Comelec record and Certificate of Baptism and the
name ELIAS BAGI RAMIREZ with date of birth DECEMBER 16, 1969 as appearing in
my own Certificate of Live Birth and other pertinent papers, refer to one and the same
person.

The my date of birth be corrected from DECEMBER 16, 1970 to DECEMBER 16,
1969;

That I am executing this affidavit to attest to the truth of all the foregoing for whatever
legal intents and purposes this may serve.
