AFFIDAVIT OF CORRECTION

I, EDITHA CHAVEZ CABSABAN, of legal age, married, Filipino and a resident of

Barangay Katangawan, General Santos City, after having been duly sworn to in accordance
with law, do hereby depose and say;

That I am the same EDITHA TAMBA CHAVEZ who was born to spouses
CRISTINA TAMBA and ATANACIO CHAVEZ on April 30, 1953 at Pontevedra, Negros
Occidental;

That I was married to JUANITO CABSABAN;

That in my COMELEC record, my date and place of birth were erroneously indicated
thereat as date of birth APRIL 25, 1954 and place of birth ILOILO CITY instead of date of
birth APRIL 30, 1953 and place of birth PONTEVEDRA, NEGROS OCCIDENTAL;

That the name EDITHA CHAVEZ CABSABAN with date of birth APRIL 25, 1954
and place of birth ILOILO CITY as appearing in my COMELEC record and the name
EDITHA TAMBA CHAVEZ with APRIL 30, 1953 and place of birth PONTEVEDRA,
NEGROS OCCIDENTAL as appearing in my own Certificate of Live Birth and other
pertinent papers, refer to one and the same person.

The my date and place of birth be corrected from APRIL 25, 1954 and place of birth
ILOILO CITY to APRIL 30, 1953 and place of birth PONTEVEDRA, NEGROS
OCCIDENTAL;

That I am executing this affidavit to attest to the truth of all the foregoing for whatever
legal intents and purposes this may serve.
