AFFIDAVIT OF CORRECTION

I, FEDERICO TANO JAYSON, of legal age, married, Filipino and a resident of

Purok Matinabangon, Barangay Labangal, General Santos City, after having been duly
sworn to in accordance with law, do hereby depose and say;

That I am the same FEDERICO TANO JAYSON who was born to spouses
MATILDE TANO and JUAN JAYSON on September 14, 1959 at Carmen, Cebu;

That I was married to MA. MERCHIE SANDOVAL CANOY on April 27, 1992
at General Santos City;

That in my Comelec record, my date of birth was erroneously indicated thereat as

FEBRUARY 6, 1959 instead of SEPTEMBER 14, 1959;

That the name FEDERICO TANO JAYSON with date of birth FEBRUARY 6,
1959 as appearing in my Comelec record and the name FEDERICO TANO JAYSON
with date of birth SEPTEMBER 14, 1959 as appearing in my own Certificate of Live
Birth and other pertinent papers, refer to one and the same person.

The my date of birth be corrected from FEBRUARY 6, 1959 to SEPTEMBER 14,

1959;

That I am executing this affidavit to attest to the truth of all the foregoing for
whatever legal intents and purposes this may serve.
