TENDER OF SUBSTITUTED SERVICE

I, Atty. Mikhael O. Santos, of legal age, Filipino, and a resident of Catarman, Northern
Samar, Attorney III at the Provincial Legal Office of the Province of Northern Samar do
hereby aver and state that:

1. That on ______________________, through the process of the Provincial Legal Office a

copy of a Notice of Hearing of Administrative Case No. 2019-001 was attempted
to be served to Mr. Bernardo E. Raquiza at his stated residence;

2. That such service of notice failed due to the absence of Mr. Bernardo E. Raquiza;

3. That service of such notice was also made through registered mail as evidenced
by Registry Receipt No. ________________;

4. That service of the notice within reasonable time cannot be realized due to the
continuous absence of the respondent at his stated residence and the absence of
a return card from the Local Post Office;

5. That as to not to impede the proceedings of this Honorable Office and

considering that personal service cannot be effected despite diligent efforts,
herein counsel hereby resorts to substituted service of notice;

 6. That the person to whom such Notice of Hearing is served is of sufficient age and
discretion;

7. That the person to whom such Notice of Hearing is served resides at the
respondent’s place of residence

WHEREFORE, counsel hereby serves this Notice of Hearing through substituted

service based on the above stated reasons.
