S – The client came readily to the virtual meeting well-groomed and casually dressed. Her
speech was steady, and her thought process appeared to be relevant and coherent. The client
underwent an eye surgery for her vision that ended up successful. She said she was happy to
have done it and felt good during the healing process. The client described she was feeling very
optimistic in the goals we had set. She has taken time off work for rest with her surgery and
registered for less classes in her fall semester. The client spoke with confidence and clarity
while describing how she had been focusing on the positives and had gratitude for the COVID-
19 restrictions easing up.

O – The client seemed very confident in terminating our counselling sessions. She understood
how to listen to her body when rest was needed. The client said that when it is hard to get out of
bed or felt drained, these are indicators that she is taking on too much. The client appears to have
the awareness and mindfulness of allowing her emotions and vulnerability to be expressed. The
client shared, when she expresses her feelings she feels more at peace, relaxed, and less stressed.

A – The client seemed to receive our sessions well and put the tools we use, into practice. She
had demonstrated what it looked like to have a healthy balance in her life, and to manage her
time efficiently. She has taken time for rest after her surgery, set boundaries with her boss at
work, establishing self-care, getting outdoors, and applying gratitude. With the clients’ actions
being matched in her words, I believe the client is ready to terminate our sessions. She is an
advocate for herself and has been able to identify her own needs without me guiding her this
way. When discussing what she will do to continue the work in the sessions, she was able to
come up with an action plan in following through and holding herself accountable.

P – This was the client’s last session with me today. We worked on summarizing the work we
have done throughout our sessions, supports the client has outside of counselling, and the
results and accomplishments the client has progressed. Although our sessions have come to an
end, I recommended her specific university counselling services and extended my services in
the future if ever needed. The client was provided a survey on her experiences throughout the
session that she has completed.
