AFFIDAVIT OF CONSENT
I, FILA MAE B. CANTO, of legal age, married, Filipino and resident of
VSM Phase II, Blk., 12 Lot 9, Grace St., Brgy. San Isidro, General Santos City,
after having been duly sworn to in accordance with law, do hereby depose and
say;

1. That I am the biological mother exercising maternal authority over

MATTHEW FRANCIS BEQUILLA CANTO who was born on
April 21, 2008 in General Santos City Hospital, General Santos City,
South Cotabato;

2. That I give my maternal consent to the said MATTHEW FRANCIS

BEQUILLA CANTO to travel abroad together with her grandmother
MARINA RUALES BEQUILLA on October 18-30, 2019 at
Thailand;

3. That I am executing this affidavit to attest to the truth of all the

foregoing for the purpose of obtaining a Permit to Travel Abroad
from the DSWD Office, General Santos City and for whatever legal
intents and purposes this may serve.
