SUBJECT: Meeting Cancellation Announcement

We regret to inform you that this rescheduled Home Health, Home Services, and Home Nursing
Advisory Board meeting has been cancelled for November 30, 2017 from 10:30 a.m. to 12:30
p.m. due to lack of meeting a quorum to hold a Board meeting. We regret any inconvenience this
cancellation may have caused.
The next scheduled meeting is for Wednesday, January 10, 2018 from 10:30 a.m. to 12:30 p.m.
locations of the video conference centers are: 525 W. Jefferson St., 4th Floor, Springfield, 122 S.
Michigan Ave, 7th Floor, Room 711, Chicago, #22 Kettle River Drive, 2309 W. Main St., Marion,
and 4212 St. Charles Road, Bellwood. Please make preparation to attend this meeting and
mark your calendar accordingly.
See attached a copy of the Future Schedule Meetings for 2018 for this Board, and note the
conference locations that will be available for these meetings. Please mark your calendar
accordingly and plan on attending.
