Case Scenario

Case Scenario 2 Pediatric Bronchopneumonia

Rina, a 2-year old child was confined at Bulacan Medical Center for 2 days now, according to her mother she
had cough for two weeks, on and off fever, shortness of breath, and loss of appetite. “Dalawang linggo ng may ubo ang
anak ko at nahihirapang syang huminga. Wala din siyang gana at pabalik balik ang lagnat” as stated by the mother.
This patient was diagnosed to have bronchopneumonia by her attending physician.
During nurse’s rounds, Rina is crying and irritable, sometimes expectorate thick phlegm, and doesn’t want to
answer when ask, breakfast is served but didn’t touched. She has an IVF of D5 0.9 NaCl 500ml at the left hand,
regulated at the rate of 60 gtts/min, and an oxygen inhalation via face mask.
Physical assessment was done by the nurse with the following result: Temp. 38.9°C; pulse rate 122 beats/min;
respiratory rate 42 breaths/min; rales sound was heard during auscultation, pale skin color, dry mouth, and
occasional productive cough.

Doctor’s order was noted as follows:


⮚ Paracetamol syrup, 1tsp q 4, PRN for fever above 38 degrees centigrade
⮚ Salbutamol nebulization, q 6 hours.
⮚ Guiafenesin 1tsp TID for 7 days.
⮚ Tetracycline 100mg/ ml every 12hours X 7days
⮚ O2 inhalation PRN
⮚ IVF Ds .09 Na Cl 500 X 8hrs

Level II 2nd Semester SY 2020-2021


Given the medical situation above, prepare one (1) nursing care plan which best address the priority problem
of the client and develop a drug study form for each medication mentioned in the case.
