Case Scenario
Case Scenario 9 Meconium-Stained Baby
Baby boy ASDF, at 40 weeks of gestation, was delivered through Normal Spontaneous Delivery (NSD) at
exactly 7:30 am. Upon delivery, he exhibited good cry, positive activity and grimace but with slightly cyanotic upper
and lower extremities. In addition, his vernix caseosa was tinged green, indication of meconium stain.
He was dried thoroughly and secretions were suctioned on the nasopharynx and mouth. Then routine
newborn care and cord care was rendered by the nurse on duty. Vital signs taken as follows: heart rate is 165 beats
per minute, respiratory rate is 51 cycles per minute and temperature which is 35.2 degrees Celsius. Anthropometric
measurements were taken as well, Head circumference is 35 cm, chest circumference is 33 cm, abdominal
circumference is 32 cm, length is 51 cm and he weighs 3.4kg.
Immunization such as Vitamin K 0.5 mg given IM on right thigh, Erythromycin eye ointment was administered
generously OU. Thirty minutes after delivery, the baby was cold to touch and is shivering with a slightly pale in
appearance. He was ordered with Amikacin 30mg IV OD and Ampicillin 100mg IV BID.
