    Our records reveal that you failed to report for work and was absent for
    a long period from ____________________ to _________________
    without official leave of absence. In this connection, unauthorized
    absence or absences without leave (AWOL) constitutes gross and
    habitual neglect of duties, which is ground for termination. You were
    likewise invited to a hearing/ conference however, you also failed to
    attend the same.
