I arrived to work and began working on appointment sheets that had not yet been finished. I
checked the nurse phones again and returned any phone calls that needed to be returned. After
this, I worked on sending referrals to doctors and calling the patients to follow up. For the rest of
the afternoon, I did paperwork, scheduled appointments, and checked all of the messages and
returned them.
When I got to work, I began by calling all the places where I had sent referrals to follow up on
the information. After this, I finished up any appointment sheets that I had from the following
day. After this I spent almost the rest of the day making care calls to patients in our CCM
program and documenting the information into our system for that program. Upon leaving work,
I had to travel to Dr. Chronos’s house in order for him to sign important patient/employee
documents for another office location in Athens. These documents had to be hand- signed and
given back to the manager in order for the employees to continue working within a healthcare
system.
** Originally, I did not include this in my journal because I was unaware that I was
allocated work hours for this errand. My supervisor did not clock me out until I left Dr.
When I arrived at work, I began the day by going around the office and making sure that
everything was stoked properly and looked presentable. After this, I did some scanning that was
in the front office and picked up the mail. I sorted the mail and delegated it to those it needed to
go to. After this I made care calls for a couple of hours and logged the information into the
system. After this, I began working on strategies and taking notes on how to improve our CCM
program.
When I arrived at work, I finished scanning a few documents left from the previous day. After
this, my supervisor gave me a list of about 300 patients that needed to be called and rescheduled
because one of our providers is leaving the clinic on maternity leave. I worked on this list for
most of the day. After this I worked on care calls and scheduled appointments from that day and
On Friday, I worked at another Lake Country Medical Clinic located at Presbyterian Village in
Athens, GA. When I arrived, I cleaned and set up the office, as it is very small, for what was
needed for that day. Throughout the day I greeted patients who stopped by and gave tem
information about the new clinic. After this, I scheduled appointments from the previous day and
checked all the messages. Ending the day, I organized, scanned, and charted patient information
