establish or build a connection for the fiber wire on my property. This is to give service to

This established connection will only be use as a connecting terminal for the internet fiber
connection, and that there will be no other use or that it will not cause an unpleasant effect to my
property.

This letter is signed upon the request of ____________________, in compliance to the requirement
of Globe Telecom’s service installation.
