DISCHARGE PLAN

Medication
 Ensure that the patient is capable of following medication instructions
Rationale: To achieve the medication's therapeutic impact and avoid further problems and
dangers
 Instruct the patient to take all the prescribed medicine at the right time as directed by the
physician
Rationale: Medications should be taken regularly to make sure that it has an effective amount of
drug in the body.
 As per physician’s order advice the client to take in extra vitamins such as multivitamins like
iron, calcium and other basic vitamins
Rationale: To ensure that the client maintain their needed nutrient and replenish the client’s body
and immune system.

Activity
 Instruct the patient to avoid hard lifting, excessive stair climbing and intense activity
Rationale: Increased activity may cause cramping and bleeding
 Advised the patient to get as much rest as she can
Rationale: Rest promotes complete recovery to the client and gain energy
 Advise the client to skip intercourse with her partner till the time you have totally recovered
Rationale: Healing might get delayed with intercourse
 Advise the client to use hot bags if she have cramps in the body
Rationale: It helps relieve pain and cramps

Treatment
 Emphasized the importance of regular follow up check-up and as instructed by the physician
Rationale: To make sure that the client has full awareness and knowledge about the condition.
This will help to monitor her condition.
 Advise the family members to seek medical advice if any abnormality arises
Rationale: To prevent further complications and have the client monitored until full recovery is
attained
Hygiene
 Instruct the patient to avoid the use of tampons or douche
Rationale: To promote discomfort to the client since having things attached or inserted in the
vagina will cause pressure and discomfort. This may also result infection to the area.
 Instruct the client to wear loose fitting clothing that will not rub the incision area.
Rationale: To prevent irritation from the garments
 Instruct the patient not to use any ointments or lotions directly on the incision unless instructed to
do otherwise.
Rationale: They can damage the skin in the wound and delay healing
 Check the area of your incision every day for redness, swelling, drainage or wound opening
Rationale: To reduce the risk of infection

Outpatient
 Encourage patient to follow scheduled check-ups.
Rationale: To see how the patient is doing and ensure that there aren’t any complications
 Follow the physician’s order or recommendations before returning to normal activities
Diet
 Instruct the client to increase fluid intake.
Rationale: Staying hydrated is essential to your health it also helps to avoid constipation.
 Ensure that the patient’s diet includes plenty of protein, iron, B vitamins and calcium
Rationale: Eating the right nutrients will help the patient recover more quickly
 Instruct the patient to avoid alcoholic beverages while still taking prescription pain medication
Rationale: Alcohol drinking can worsen serious medical conditions and delay healing
 Instruct the patient to avoid smoking
Rationale: Smoking reduces blood flow which slows healing process.
Spiritual
 Encourage the client to continue to seek God’s guidance and enlightenment
 Discuss the use of relaxation/ meditative activities (yoga, prayer)
Rationale: It promotes general well-being and sense of connectedness with self, nature and
spiritual power
 Support Patients Within Their Own Faith Tradition
 Share spiritual thoughts that can encourage the patient
