OBJECTIVE:
AFTER 2 YEARS OF THE ACTIVITY THE COMMUNITY PEOPLE WILL BE ABLE TO UNDERSTAND THE IMPORTANCE OF
UNITY AND APPLY SELF DISCIPLINE AND ABIDE TO THE RULES OF SCHOOL TO PREVENT PROBLEMS AND BRING
PEACE TO THE SCHOOL
INTRODUCTION:
THE ANNAFUNAN EAST TUGUEGARAO SCHOOL IS LOCATED NEAR THE HIGHWAY ROAD OR MAHARLIKA IT IS
COMPOSED OF KINDERGARDEN UNTIL GRADE 6, IT IS A PUBLIC SCHOOL WHICH IS GOVERN BY THE PHILIPPINE
GOVERNMENT. THIS SCHOOL IS A PEACEFUL SCHOOL THEY PROHIBIT FIGHTS AND DISCIPLINE THEIR STUDENT WELL.
THEY HAVE RULES THAT IS STRICTLY IMPLEMENTED. THEY MAINTAIN CLEANLINESS AND ENVIRONMENT.
PROJECT RATIONALE:
TO ENSURE THAT THE STUDENT WILL ACT ACTIVELY TO THE ASSIMILATION IN COMMUNITY ORGANIZATION TO
IMPROVE AND TO BRING UNITY IN THE SCHOOL.
AND TO BE AWARE OF THE ADVANTAGE OF HAVING PEACEFUL SCHOOL.

ACTIVITY 5
1. Both the health care process and the Neighborhood health care process are workable and require time and effort, but their primary
aim is to encourage understanding of health and to assess client health status during the implementation of a program. Both refer to
both in terms of community health learning by empowerment or action processes about how to deal with health threats / issues in the
community health care environment
2. Our community class instructor formed a small group and assigned us to our perspective area where in we are about to conduct our
activity, we formulate questionnaire for guide questions to use in gathering information. then we assess on the area where there is
need for help. As a nurse we always bring our PHN Bag to for any emergency while doing our assessment.
3. The problem we encounter was: people are not cooperating, no peace, no disciplined and time management but it was all managed
when the student leaders assigned was oriented on the things to be done, they help us to make peace and let the people cooperate and
through that our scheduled of time was fixed.
