AFFIDAVIT OF CLOSURE/TERMINATION

I, BERNADITA MIRONTOS, of legal age, Filipino, married and a

resident of Brgy. Garang, Tangub City, Misamis Occidental, Philippines,
after being duly sworn in accordance with law, hereby depose and state:

1. That I am proprietor/owner of Sari-sari Store with business

name Bernardita Mirontos Sari-sari Store located at Brgy.
Garang, Tangub City;

2. That I am executing this Affidavit of Closure in my said

capacity;

3. That during the month of June 2020 the said business was
closed and in no longer operating until this very present
time;

4. That the reason of termination of said business is that I

can no longer look after the store because I decided to sell
goods at the public market;

5. That I hereby execute this Affidavit to attest to the truth of

the aforementioned facts and for any other legal purposes
that this Affidavit could serve.
