COVID 19 Vaccination represents our greatest hope to combat the devastating pandemic. Vaccine allergy have been reported since the
initiation of the vaccination campaigns but exceedingly rare. The estimated the rates of anaphylactic reactions in mRNA BNT162b2
vaccines was 11.1 per million administered. Inactivated vaccine CornonaVac is likely having lower severe allergies than this figure.
Although rare, such rates could have importance due to the fact that a large majority of the world population will be subjected to
vaccination and perhaps recurrent basis. It is important to engage with patients, communities, public health officers and frontline clinicians
on this topic. There are many types of heterogeneous allergic reactions that may lead to deferral of the first or second dose. In fact, most of
the common allergies such as atopic dermatitis, allergic rhinitis, asthma, food, venom, latex insect allergy are not contraindications. HK
Health regulators and allergists have advised that any subject with a previous history of allergy to drugs or any component of the vaccines
should not be vaccinated. In this presentation, I provide an updated revision regarding the anaphylactic reactions described for COVID 19
vaccine and role of testing of different excipients in the vaccines with the potential to elicit systemic allergic reactions such as polyethylene
glycol (PEG), polysorbates and the possible immunological mechanisms involved. The approach for prevaccine testing and
aftercare/testing for the first dose reaction is also discussed .
