Dear Ms Houlihan,

This is to urgently transfer Ms Yi, who tested positive for COVID-19 after a travel exposure from a high-risk region. She
requires intensive care and support.

At around 7 a.m., Ms Yi presented with breathing difficulties and wheezing due to hypoxia, cough, and hyperthermia.
She was alert and oriented but unable to ambulate. In addition, no swelling or tenderness in the extremities were
noted. Several diagnostic tests ,including coronavirus corona virus and flu tests, were done and yielded positive results.

protective equipment. And Administration of normal normal saline 500 ml intravenously along with supplemental
oxygen through a non-rebreather mask nonrebreather mask were initiated. But due Due to agitation, an increase in
respiration, and low oxygen saturation of 86%, she has been was intubated and connected to a mechanical ventilator
successfully.

Currently, Ms Yi’s oxygen level has improved to 92%, and IVF, post- intubation ,and sedation care were done.

As a care plan, it would be greatly appreciated if you could to accept Ms Yi in the ICU for continuous care and
management.

Ms. Yi, who has been diagnosed with a positive screening for COVID19, is being transferred
urgently into your unit following her intubation. She requires critical care and management.
 
Ms. Yi presented in the Emergency department today with difficulty breathing, wheezes, cough,
fever (39.2 c), and tachycardia. She was oriented but unable to ambulate due to weakness. At
7:10 am, she was transferred to an airborne isolation room and was commenced on oxygen
therapy via a non-rebreather mask and 0.9NS IV 500ml as bolus administration. Investigations
such as ECG, X-ray, blood tests, as well as COVID19 and flu swab, were done. After 20
minutes, she remained tachypneic, but her O2 saturation increased from 84% to 90% with
supplemental oxygen. Then, she deteriorated, as evidenced by agitation and hypoxia, along with
increased respiratory rate (30). Consequently, she has been intubated and ventilated with 100%
oxygen at 7:55 am, saturating on 92%. IV bolus and post-intubation care, including sedation,
have been commenced. 
 
Ms Yi went on a business trip in a region known to have apparent COVID-19 cases two days
ago. She has had asthma since childhood. Her brother, David Yi, has been notified about her
current condition. 
 
As a care plan, it would be much appreciated if you could admit Ms. Yi to your unit
immediately for ongoing treatment.
