Discharging to Inpatient Hospice Background: When discharging a patient to hospice there will be two options: Discharge Hospice- Home or Discharge Hospice- Medical Facility. If a patient is staying at our facility for inpatient hospice or transferring to another medical facility for hospice care they must be discharged from the original medical account. If staying at our facility a new account MUST be created for their Hospice stay. Choosing the appropriate disposition is key when facilitating hospice benefits for the patient along with the financial impact for the hospital.

Admitting to New Account • Follow facility protocol to get patient readmitted under NEW Hospice account • Complete admission and assessment per facility protocol/policy o Remember patient will need a new chart created & a new armband placed on patient for medication scanning • If patient expires, discharge disposition is then EXPIRED on the hospice account
