• Correctly discharge a patient into a inpatient
hospice account

Discharging to Inpatient Hospice
Background: When discharging a patient to hospice there will be two options: Discharge Hospice- Home
or Discharge Hospice- Medical Facility. If a patient is staying at our facility for inpatient hospice or
transferring to another medical facility for hospice care they must be discharged from the original medical
account. If staying at our facility a new account MUST be created for their Hospice stay. Choosing the
appropriate disposition is key when facilitating hospice benefits for the patient along with the financial
impact for the hospital.

Discharge Disposition
• From Status Board click Discharge, follow the prompts and add items according
to the Provider order
• Complete Discharge Disposition on existing account
o Discharge to hospice - Home
o Discharge to hospice - Medical Facility  For Inpatient Hospice choose this

Discharge Instructions and Forms

• Under discharge plan at the top complete:


o Disposition
o Instructions
o Forms
• Finalize discharge Medicine Reconciliation if not already completed
o This allows the medications to be pulled correctly over to new account

Discharging a Patient out of Meditech

• Ensure you chose the correct facility when choosing location of discharge upon removing account
• From print screen follow blue highlights

• Choose Hospice-Medical Facility

After selecting Hospice Medical


Facility choose the appropriate GIP
Hospice for patient’s being
admitted to your facility

Admitting to New Account
• Follow facility protocol to get patient readmitted under NEW Hospice account
• Complete admission and assessment per facility protocol/policy
o Remember patient will need a new chart created & a new armband placed on patient for medication scanning
• If patient expires, discharge disposition is then EXPIRED on the hospice account

Complete in Discharge Disposition


Complete when removing patient from system
