The radiographer performing the examination completes the remaining blocks in the top section. The film interpreter fills out the bottom portion for results and types of defects. A legend for defect codes Is given in the bottom left corner. The film interpreter must identify each type of discontinuity, list the size and location of the discontinuity and accept/reject the component. If he makes an error in documentation, he needs to draw one line through the mistake and initial and date next to the line. Correction fluid is never to be used on inspection documentation.
