Purok Elena, Barangay Sudlon, Molave, Zamboanga del Sur. After having been duly sworn in
accordance with the law, do hereby depose and state that:

1. I am applying for Teacher at Parasan National High School, Division of

Zamboanga del Sur;

2. I am executing this omnibus sworn certification to assure authenticity and veracity of

the following documents:

2.1. CSC Form 212 (revised 2017) in two copies with the latest passport size
ID pictures
2.2. Certified copy of Transcript of Records
2.3. Certificate of General Weighted Average (GWA)
2.4. Authenticated Report of Rating (2 copies)
2.5. Certificate of specialized training program attended
2.5.1. COMPUTER LITERACY TRAINING PROGRAM in Microsoft
Word, Excel, PowerPoint and Publisher.
2.6. Certified copy of Voter’s ID
2.7. Photocopy of Police Clearance
2.8. Photocopy of NBI Clearance
2.9. Omnibus certification of authenticity and veracity of all documents
submitted, signed by teacher applicant.

3. By executing the OMNIBUS CERTIFICATION AND VERACITY OF

DOCUMENTS, I hereby authorize the Department of Education, Division of
Zamboanga del Sur through the Head Teacher, to verify the authenticity of the above
mentioned documents.
