1. What is work at height activity, and its associated hazards?

Ans: Work at height - Any activity involving working at elevation greater than 1.8 m (6 ft.)
Hazards- Fall of objects /personal, slip trip and falls at elevation, contact with overhead power lines,
exertion due to climbing.

2. What is confined space, and its associated hazards?


Ans: Any space which is not made for human occupancy and by virtue of it has following characteristic’s.
1. Lack of oxygen (<19.3 %) or oxygen enrichment (>23 %)
2. Flammable atmosphere (accumulation of gases)
3. Presence of hazardous gases/Substances.
4. Limited access and ingress
5. Poor illumination
6. Lack of ventilation
7. Energized equipments (if any)
8. Internal obstructions.
9. Residual gases
10. Noise
11. Temperature extremes

3. What are excavations and its types?


Ans An excavation is any man-made cut, cavity, trench, or depression in the earth's surface formed by earth
removal.

4. What are arrangements for confined space preparation?

Ans: Prior to entry into confined space


- Risk assessment for confined space
- De energized all energy sources (Mechanical, electrical, hydraulic /pneumatic)- LOTO
- Purging (Displacement of hazardous gases by introduction of fresh pressurized air)
- Draining (Removal of hazardous waste)
- Air quality testing
- Mechanical Ventilation by blowers (Introduction of fresh air)
- Gas testing
- Permit to Work systems
- Authorized and competent workers & certified standby man.
- Emergency and rescue arrangements
- Obstruction free access
- Sufficient illumination
- Communication methods (Radio, telephone, verbal)
- PPE recommendations.

5. What are electrical hazards? also what are additional hazards for electrical activity at elevations?
Ans: Burns & Shocks,
At elevations – Falling personnel/objects due to contact, slip & trips., contact with overhead livewires.

6. What is RISK? describe the Risk control or hierarchy of controls in Work place?
Ans: RISK – It is a likelihood that a hazard can cause harm in combination with severity of injury

7. What are the elements /Modules of Health & Safety Management systems?
Ans: POLICY, ORGANISATION, PLANNING, IMPLEMENTATION, MANAGEMENT AND REVIEW.

8. What is emergency rescue plan?


Ans: It’s a formal document that facilitates and organize people actions during rescue activity

It should Include- Activity and location details, equipment details, scope of work, elevations, rescue
equipments, lighting arrangements, rescue personnel’s identifications, communication methods, mechanical
lifting etc., breathing arrangements like SCABA.

9. Emergency evacuation plan?

Ans: It’s a formal document that facilitates and organize people actions during evacuations

It includes - Activity and location details, equipment details, scope of work, communication methods,
emergency route, emergency contact nos., signages , assembly point area details , responsible person.

10. What is LOTO?

Ans: LOTO (Lock Out Tag Out) is the physical restraint of all hazardous energy sources that supply power
to a piece of equipment.
(LOTO also includes applying a Warning Tag on the physical restraint device. This documents the
Authorized LOTO personnel and the date. LOTO operations must be done on all equipment, machinery or
system Shut Downs before Authorized Personnel can perform repairs or service.
Mechanical energy (Hydraulic /pneumatic lines – Blinding, valve lock outs)
Electrical energy (circuit breaker lockouts, plug and wall switch lock out)

Special procedures for "group lock out" must be included in the mandatory LOTO procedure. Special
"group lock out" devices are available that can hold multiple padlocks. This assures each person’s safety
until the work by all Authorized Personnel has been completed. All personnel participating in a "group lock
out" must be trained on the LOTO procedure and the special "group lock out" procedures.

12. What is critical lifting?


Ans : A lifting activity which has following characteristic
- Lifting in night shift
- Lifting weight more than 75 % of SWL of crane
- Lifting near hydrocarbon facilities
- Lifting in tandem (Multiple cranes operating in same area)
- Load maneuvered outside of the crane operator view

13. Difference between fall prevention and fall protection ?


Ans : Fall protection – Measure to prevent falling

14. What is hot work requirements?

Ans: Hot Work- Any activity generates ignition sources, flames, sparks, fire etc.
Eg. Welding, cutting, grinding

Risk assessment- LOTO, de- energized/purged equipment, blinding, lifting plan etc.
Special protection- like SCBA, gloves, respirator, GFCI, Air movers etc.
Fire protection- Water hose, fire blanket, and Fire extinguisher
Fire watchmen details if required
Gas tests type and frequency of tests e.g. Oxygen 19.5 to 23.5 , combustibles < 2 %, Ammonia <
25 ppm , CO < 25 ppm, H2S < 10 ppm, HF < 3 ppm , S02 < 2ppm etc.
Issuer details, receiver details, field operator details,
PPE recommendations and other controls, welding equipment’s inspections, gas cylinder controls
