SBF Acknowledgement of Receipt and Certification of Guidance

The purpose of this acknowledgment and certification is to verify that both parties,
Endeavors and the Applicant, have discussed main points regarding the SBF Program
application process and eligibility overview. To verify, please sign and return this

I, Noemi Cordero Santiago, certify that I have met and been informed by Wilkyns Nunez Pena
from Endeavors a PRHOD Procured Contractor on the basic qualification and eligibility
requirements for the Recovery Grant Application under the SBF Program of the CBDG-DR.

During the meeting held on 5/12/2021, I discussed the business history, ownership, financial
overview, damage related to Hurricanes Irma and/or María, and plans for the future of my
business. The Case Manager explained SBF Program eligibility details and how Duplication
of Benefits (DOB) are calculated. The Case Manager also explained the importance of
providing information to the SBF Program on all funds received by the business for the same
recovery purposes. The Case Manager informed me of the requirements to prove proper use
of any awarded funds and the possibility of recapture if the business does not comply with the
Program closeout requirements. In addition, the Case Manager provided guidance as per
funding options and different alternatives as an applicant within the SBF Program.
