REASON FOR OVERLOADING/SIMUL

I am Gatdula, Frenz Jerome Espiritu, a 5 th Year BSCE student and as a graduating student I want to finish
my academics in the fastest way for me to achieve my goals outside this institution so I’m expecting you
to approve my overload and simul which is 14 units enrolled and 9 units more to add. I am very grateful
if I have the chance to take all the subjects and graduate this 2 nd semester, A.Y 2020-2021.

WAIVER OF OVERLOAD -- I hereby attest that the course indicated above were the only courses left
for me to qualify for graduation for the program. However, in case my records at the Office of the
Registrar indicate otherwise, I am willing to drop the Design Course from my study load for the
semester.

WAIVER OF SIMUL: I fully understand that if ever I failed the prerequisite/s course/s, the higher
course/s will not be given credit regardless of the grade/s obtained.
