Instructions for Sophia University Application

Follow these instructions line by line when filling out your Sophia University Application Form .

ATTENTION: If you are in one or more of the following applicant categories, please notify your CIEE Study Abroad Advisor in
Portland ASAP.

1. Japanese/US Dual Citizen Applicants (you should enter Japan with a Japanese passport)
2. If you have attended other colleges or universities in the past (you will need to provide transcripts from those schools)
3. Non-Native English Speaker Applicants (TOEFL/ IELTS score may be required)
4. Applicants with Disabilities or Special Needs

Quick Notes:

* Click on the second, orange tab below ("SOPHIA APP") for the application form.

* This form is in EXCEL format and it needs to stay in EXCEL format so we can merge it onto the official application
paper.

* Please type in all capital letters (turn on the CAPS LOCK key.)

* DO NOT fill in the fields highlighted in LIGHT ORANGE. If a field already has information in it, DO NOT REPLACE IT.

* Enter ALL schools you have attended SINCE ELEMENTARY SCHOOL and be sure to include:
- Location - State, Country
- Dates - YYYY/MM - YYYY/MM (estimated dates are OK)
- Main Language of Instruction of all courses

ACADEMIC YEAR and SEMESTER: Write the year and the semester (FALL or SPRING) you are applying for.

PERSONAL DATA
LAST NAME, FIRST NAME, MIDDLE NAME: Your full name must be written exactly as it appears on your passport. Do not us
nicknames or unofficial names. If your middle name appears in your passport, you must include it on your application form

NAME IN JAPANESE/CHINESE CHARACTERS (if applicable): For those who intend to use a passport issued by Japan, China,
Hong Kong, Taiwan, or Korea. Please write your official name in Chinese (kanji) characters in this box.

LAST NAME, FIRST NAME IN KATAKANA CHARACTERS (if known): If you know your last name and first name in Katakana
characters, please write them in this cell. If you do not know, please leave this cell blank. If you do not enter Katakana for yo
name, CIEE resident staff will do their best to choose the most appropriate Katakana for you. If you feel that the pronunciati

you hold a U.S. passport, your nationality would be 'USA'. Do not write your racial or ethnic background such as 'Chinese-
American', etc.). If you are a dual Japanese/US citizen, write "Japan" as your primary nationality and "USA" as your
secondary nationality and contact your CIEE Advisor for additional information and instructions.
COUNTRY OF CITIZENSHIP (SECONDARY NATIONALITY): For those who hold citizenship in a second country. This requires th
country name where your secondary passport was issued. (i.e., if you also hold citizenship in Australia, write "Australia." If y
are a dual Japanese/US citizen, write "Japan" as your primary nationality and "USA" as your secondary nationality and
contact your CIEE Advisor for additional information and instructions.)
EMAIL: Enter the email address which you used for your CIEE Application.

LOCATION (State, Country): Enter the state and country of the school you are currently attending.

GRADUATION (EXPECTED) YYYY/MM: Enter the year and month in which you are expected to graduate.

Please list all schools attended from elementary school through university. Include universities/colleges that you have
attended in the past (there is no need to write your current school, as you have already written this in a previous cell). Write
both the month and year (YYYY/MM) of the starting date and ending date for each school. Start at the top with the first
elementary school you attended and finish at the bottom with the most recent school PRIOR to your current school.
