The activity aims to review and assess the performance of school heads in the different key result areas based on their commitments and measure which will also be the basis of rewarding and development planning activities of the division. 3. The generic OPCRF template crafted by the Regional Office released though RM No. 33, s. 2021 and Division Memorandum No. 82, s. 2021 will be used for SY 2020-2021 performance evaluation. 4. The rating for a particular task accomplishment should be supported with reports, documents or any output as proof of actual performance (MOVs). In the absence of said bases or proofs, a particular task shall not be rated and shalll be disregarded. 5. After the evaluation, three (3) copies of the OPCRF will be submitted to the Office of the ASDS. 9. Strict compliance to health protocols will be observed such as wearing of face mask/s and face shields, use of alcohol, disinfection of documents, physical distancing, and submission of health declaration in the conduct of the activity. 10.
