Field of study/undergrad major

A. Construct a Table of Specifications (TOS) based on your sample test questionnaires.

B. Develop sample questionnaire for Summative Test applying the Bloom’s Taxonomy of
Learning. (Observe the principles in constructing a Traditional forms of Assessment)

Note: Observe the proper format for a Summative Test (Test I-IV, 15 items each
and for Test V any of the following, Essay Test or Problem solving)

II. True or False
III. Matching Type
IV. Completion Type
V. Essay test (2 questions)
Problem solving (3 questions)
