President Rodrigo Duterte has extended the
declaration of a state of calamity throughout
the Philippines for one more year due to the
Covid-19 pandemic. The President
admittedly stated that though the
government are doing significant changes to
fight the spread of virus, the Covid-19 cases
continue to increase in our country. Despite
the intervention and cooperation of all
sectors the death toll still rapidly increase.

(e.g. enrolment form, deposit or withdrawal
slip, etc.) available in your locality. Paste in
your notebook. Using your chosen form,
specify what specific details it asks from its
clients.

LESF. It is a standardized form for students
who wanted to enroll. The information that
need to fill out in this form are the basic
students’ information such as name,
address, birthday, birthplace, LRN,
information about their parents and its
educational backgrounds. It is also a survey
form to know the availability of manpower if
the students’ were in modular distance
learning.
