That this letter is intended to seek assistance to allow us to release shipment as this is for
personal use only and in good faith is not intended to defraud the Government or bypass its rules and
regulations. In addition, the undersigned is willing to pay the corresponding amount of duties and taxes
due on the said shipment.

I undertake to be responsible and accountable to comply with all rules and regulations prescribed
by the Bureau of Customs governing the release of our importation.
