Inspection: Preliminary

Visual Inspection was carried out. Following are the major observations:

Observations:
1) Yellowish appearance was observed around repair weld performed at leaked nozzle 1N
weld inside vessel in previous turnaround (Pic.1).
2) Grossly overall condition of the vessel was found satisfactory.
3) All circumferential and longitudinal welds of shell were found in good condition.
4) Thermocouple was found intact.
5) Slight pitting on manway cover sealing face observed in Turn-around 2014 & 2017 was
found as such. (Pic. 2).

Recommendations:
1. Power brush cleaning of nozzle 1N weld from inside and its reinforcement plate welds from
outsides and to be offered for DPT.
2. Power brush cleaning of all nozzles to be performed for DPT.

3. Perform thickness monitoring of heads, shell courses and nozzles as per plan.
