AFFIDAVIT OF UNDERTAKING

I, MANUEL P. SYCIP, of legal age, married, Filipino, resident of 3 rd road Puentebella Subd.,
Brgy. Taculing, Bacolod City and with business address at Brgy. E. Lopez, Silay City, after
having subscribed to an oath, depose and say:

1. That I am the proprietor of Manuel P. Sycip ISAG, a duly organized domestic business
under the laws of the Republic of the Philippines;
2. That the said business is currently processing the new Governor’s Permit to extract
sand and gravel materials in Lot685-B (Portion), Brgy. E. Lopez, Silay City, Negros
Occidental, Philippines;
3. That there is no opposition with the aforementioned applied area from the nearby
residents and owners of the adjacent properties and if Found later on to be having
such opposition, the City has the right to recall such permit;
4. That if the said permit was found to be in conflict or overlapping with the present
and future projects of the City, the same shall be recalled and cancelled;
5. That any violation of the conditions set forth the attached Environmental
Compliance Certificate and the City’s Environment and Natural Resources Office
during the duration of the permit shall be a ground for revocation;
6. That I execute this Affidavit of Undertaking in compliance with the Requirements set
by the legal Office of the City of Silay in issuance of the City Resolution endorsing the
my New Industrial Sand and Gravel Permit.
