 Guidance on admissions to Bernard Mizeki College

FORM 1

Admission into Form 1 is dependent upon the following:

i. A satisfactory report from the Head of the applicant’s junior school.

ii. A progress report of the applicant from Grade 6 (2nd & 3rd term) and Grade 7 (1st term).
iii. The deadline for applications is 30 August in the applicant’s Grade 7 year.

The application should include the following:

i. A US$30 registration fee (non-refundable).

ii. A certified copy of the applicant’s birth certificate.

PLEASE NOTE – We do not accept fax or email copies of the application form; please submit this original form
together with the payment at Bernard Mizeki College.
