INDIVIDUAL PERFORMANCE COMMITMENT

for SY 2021-2022

I, Henry C. Lumangtad , Master Teacher I

of Matab-ang National High School, hereby commit to perform with

competence, dedication and commitment, my duties and responsibilities,
with this performance period’s agreed objectives and targets of the Key
Result Areas of my position;

I further commit to perform the following tasks:

(1) improve the performance of my tasks, self-monitor, and whenever

needed, seek assistance and coaching from my immediate superior;

(2) conduct a mid-year review of my performance within 5 days after the end
of the First Semester with my immediate superior;

(3) conduct the yearend evaluation of my performance within 15 days after

the end of the performance period and submit my performance report
within the said period, and;

(4) develop and implement my Individual Development Plan (IDP) based on

the assessment of my work competencies and this and last year’s
performance.

Attached is my Performance Target for SY 2021-2022.
