Points of Interest for Inspectors:

1.4.5 Benzene - before Tank Entry, testing must be carried out on all tanks that had petroleum products.

1.4.6 & 1.6.4 H2S testing of all bunker tanks (before, during and after) must be carried out to confirm no H2S. vapours
with 5PPM H2S by vol. and above need managing.

1.4.6.3 Training required for all tanker personnel in safe handling of H2S (Personal gas detectors use for cargo
operations is highly recommended).

1.6.3.2 Correct use of heating coils during bunkering. Avoid empty tanks with fuel oil contacting overly hot heating
coils and producing flammable gases. Consider to be part of bunkering plan.

1.6.3.4 Headspace in fuel oil tanks to be regularly monitored for H2S. Purging may be required.

2.4.1 Two instruments (four gas) must be carried for testing prior to enclosed space entry. Plus any other toxic gas
detectors that may be needed.

2.4.1.2 FGD(Fixed Gas Detection) for pumproom must give audible and visual alarms in PR, ECR, CCR, & Bridge

4.12.2 Portable electrical equipment on wandering leads must be controlled by the SMS

4.13.4 AIS in port when working cargo should be either off or aerial isolated and appliance given a dummy load. In
port on low power also

7.2 Identification and analysis of Safety Critical Tasks (where actions and or decisions of a human are the only
protection against serious incidents) must be in place. Refer 4.2.2

7.10 Competency Management System recommended (should be required but is not mentioned in ISGOTT6 - SNN.

• Training/familiarisation of knowledge
• Practice building the skills
• Accumulating experience
• Demonstration of application in real life (under supervision - mentoring is best)

7.11 Team skills exercises – evidence needed of training such as bridge team

9.4.3 No hot work within 5m of a bunker tank bulkhead – check location of the designated hot-work space on board

9.4.4.1 Hot Work

• RA to include fire watch and evacuation in the case of an emergency


• Only one HW operation to be allowed at one time
• Personnel carrying out each task must be proved competent (see competency assurance)
• Atmosphere checks – continuous still not a requirement, incredible???!!! But is highly recommended.

9.4.5 HW company agreed PTW not required for designated hot work area (RA is still required) and non-hazardous
areas on the vessel (on board PTW and RA required)

9.5 Correct earthing of the electric welding must be clear (back to the machine not the ships structure)

9.7 Contractors must have induction and safety/toolbox meeting before working. Any PTW must also be in place.

9.8 SIMOPS (Simultaneous Operations)procedures must be in the SMS and where possible avoided. RA for SIMOPS
list in 9.8

9.9.2 Exposed transverse frames access manifold platform must have railings all around except for area of hose
connection Falling risks need to be managed. Safe access to sampling, measuring points on deck in all weathers must
be considered. (RA with safeguards)

10.1 Enclosed space entry (ESE) – only when no other alternative

10.2.ESE procedures in the SMS must consider precautions to help prevent human error (see chapter 7)

10.2.2 Attendants (ESE watch personnel) must be competent and able to demonstrate the rules of responding to a
crisis. These rules must be intrinsic to all persons on board.

10.2.3 All Enclosed spaces on the ship specific list must have controls against accidental/uncontrolled entry – ‘Entry
under a permit only’ – lockable arrangements, netting, etc. (10.5 adds secured against entry with warning signs,
entry procedures from RA posted)

10.4.2 Gases associated with BWTS rooms/spaces

10.4.3 IG rooms (including N2 generators and bottle rooms)

10.4.4 Oxygen enrichment – one they missed was the awareness of the use of resuscitators in cargo areas.

10.8 EEBD and Personal Gas Detectors are required for all enclosed space entries for each person entering

10.11.2.1 Dedicated ES rescue team – trained

10.11.2.4 resuscitator must not be locked away but must be available for immediate use.

10.12.1 Cargo Pumproom Entry - testing of the atmospheres prior entry can be done by FGD if they meet the
standard.

• Also, cargo pumproom entries must be kept to a minimum – use of CCTV helps reduce this as per VIQ.

11.1.7.1 If last cargo was sour crude then tests around the deck are required and no deck work is allowed without BA
if these tests give reading of 5ppm H2S or higher

11.1.8.1 PV breakers on deck should be marked with

• HP and LP opening values


• Date of last inspection
• Type of antifreeze and temperature to which it is effective

11.1.8.4 PS settings still not clear/safely described… If being used to monitor the correct operation of the PV valves,
then the PS alarms should be set to warn when the PV is outside its normal working settings on both the pressure
and vacuum side.

12.1.4 Pressure surges in cargo and ballast lines - no guidance on opening and closing times. Makers
recommendations? Rough guide 1-3 seconds per inch for butterfly valves.
12.1.7.7 With Static accumulator cargoes/ no IG spread loading (initial control not to exceed 1m/s a RA must be
carried out cargo operation specific. Maximum of 4 tanks to be used at this initial phase.

12.1.13.3 ??? discharging vent control procedure using IG makes sense, discharging using non-IG must stay positive?
Does not make sense. Maybe the paragraph order is wrong? Basically, it means the PV vent flow rate dictates the
discharge rate.

12.1.15.7 When cargo pump temperature sensors are fitted, action plan must be available if an alarm is activated on
what actions to carry out.

• Spray arrestors needed around rotatory cargo pump glands.

12.1.15.2 Cargo and Ballast lines drainage to the pumproom any such drainage must not contain cargo residues.

12.1.15.3 Lifting arrangements (harness and rope) from deep pump rooms to be fire retardant material.

12.1.15.4 Any electrical work conducted in PR must be with PTW and follow makers strict guidance.

• Only qualified persons may undergo any work on intrinsically safe/ explosion proof lighting.

12.1.15.6 Alarms and Trip tests of Pumproom equipment should not be limited to electrical testing and must be
thorough.

12.1.15.7 Procedures need to be in place where cargo pump bearing temperature alarms are fitted, and what actions
must be carried out when the alarm is sounded.

12.3.3. A responsible officer must supervise all tank washing operations. (not just a Pumpman/ Bosun or AB)

12.3.5 Lists the tank washing precautions required.

12.4.3 List of precautions before and during gas freeing any cargo tanks.

12.4.7 Procedures (ship specific) need to be developed on how to effectively ventilate ballast tanks prior to entry.

12.7.1 Procedures (ship specific) for the safe transfer of cargo leakage from any ballast tanks must be in place.

12.8.1 Cargo tank closed sampling procedures (ship specific) need to be in the SMS. Including the precautions with
toxic cargoes. (12.8.4)

12.8.4 Closed gauging and sampling only with toxic cargoes. If this is not possible then a risk assessment and
agreement from shore terminal is needed.

12.8.6 Cargo measuring equipment must be covered by the PMS and follow makers’ recommendations.

12.9 STS transfer guide applies

12.11 IGF code applies for vessel’s fuelled by LNG (not LNG carriers).

12.12 Lifting equipment in PMS, Training (12.12.2) In addition to standard precautions a test should be conducted
prior to the operation commencing.

Above not covering OBOs or packaged IMDG.
