ASSIGNMENT 1.

How to care for a hypertensive patient

Under the following topics

1. observation

Solution:

hypertension also known as high blood pressure is the force that a person’s blood exerts against

the walls of their blood vessels.

Usually a persons is considered hypertensive when measured with a blood pressure of systolic

reading above 140mm Hg and a diastolic reading of above 90mmHg, on two or more visits.

Nursing care for hypertensive patient are as fellows;

1. OBSERVATION

a.`` strictly check and observe the BP by making a BP chart for 30mins, 1hourly then 4hourly

respectively.
b. review laboratory investigations such creatinen ,chest x-ray, and lipid profile.

c. check and observe other vital signs such as temperature ,pulse and respiration.

d. observe patient for signs and complications such as severe headache, palpitation and pain.

e. observe patient for signs of stroke example; the way the patient speak, eat, swallow or try to

lift the limb.

2. NUTRITIONAL MANAGEMENT.

a. encourage patient to see and speak to a dietician to help get a diet plan.

b. encourage patient to avoid foods that are high in cholesterol example milk.

c. encourage patient to avoid too much of fresh meat and rather go in for plant protein.

d. advice patient to consume high fiber foods.

e. advice and encourage patient to consume or go in for a well balance diet.

3. MONITORING OF SIGNS OF COMPLICATION.

a. monitor the input and output to ascertain the kidney function level.

b. monitor and check the sugar level by conducting the following FBS,RBS to help note or

determine the presence of hyperclceamia.

c. asses patient especially on the feet to determine the presence of oedema.

d. conduct visual acuity test to determine problems of vision impairment.

4. EXERCISE.

a. advice patient against strenuous exercise

b. encourage patient to daily exercise like walking.


c. encourage patient to have enough rest and sleep.

d. encourage patient to check BP before and after each exercise session.

5. MEDICATION.

a. encourage patient to take daily routine drugs seriously.

b. teach patient on how to take drugs thus the dose and dosage.

c. explain side effects of drugs to patient and encourage them to report any side effects.

d. advise them to take only prescribed drugs by a doctor or physician and avoid self-medication .

e. advice patient to check BP whiles on medication.

6. PREVENTIVE MEASURES.

a. engage in regular exercise

b. encourage patient unrest and sleep

c .advice patient to go for regular checkup visits

d. advice against smoking and alcohol intake

e. encourage patient to avoid too much stress

7. HEALTH EDUCATION.

a. advice and teach patient on his or her condition including causes and ,preventive measures.

b. advice patient to seek immediate health care when feeling any discomfort.

c. educate patient on the importance of checking BP regularly.

d. teach patient on risk factors of hypertension.eg dizziness ,palpitation.
