RLE 107 – Family Nursing Care Plan | Group 2A 2

A. Malnutrition
Criteria Computation Actual Score Justification

1. Nature of the Problem 3/3x1 1 The presence of malnutrition among the children is identified
as a health deficit, hence it requires more immediate
intervention.

2. Modifiability of the Problem 2/2x2 2 The modifiability of the problem is high because it was seen in
the latter part of the documentary that the family has received
groceries and financial assistance, making the sources and
intervention needed to solve the problem available to the
family. Mae has also shared that they plan to start a sari-sari
store business in order to have an additional and more stable
source of income.

3. Preventive/Potential 3/3 x 1 1 Both the present and further problems related to malnutrition
can be minimized because the family can utilize the
resources provided by Mr. Mark and Ms. Rhea in providing
adequate food for the family.

4. Salience of the Problem 2/2 x 1 1 The family does recognize the problem as serious and needs
immediate attention. However, financial constraints hinder
them from getting the adequate nutritional requirements their
bodies need, especially Mae and her children as they are
considered vulnerable to diseases.

B. Poor Physical Environmental Conditions

Criteria Computation Actual Score Justification

1. Nature of the Problem 2/3x1 2/3 Poor physical environmental conditions are identified as a
health threat.

2. Modifiability of the Problem 1/2x2 1 The problem is partially modifiable because while it is true that
the family has already moved into a place that is more
conducive to live in than residing in the street, it is evident that
it is still poor in terms of environmental sanitation. Thus, there
is a need to modify their current home with regards to
cleanliness and eradication of the presence of hazards.

3. Preventive/Potential 2/3 x 1 2/3 Taking into account that the family is already living in a house
with a proper structure, the possible problems related to poor
environmental conditions such as the threat of acquiring
communicable diseases, pollutants, and weakened immunity
are decreased. However, as seen in the documentary, there is
still inadequate living space for the family members and
improper ventilation in the house they are renting.

4. Salience of the Problem 2/2 x 1 1 The family recognizes the problem as serious and needs
immediate attention. However, due to financial instability, their
desire to improve and move to a more conducive home is
being impeded.

C. Pregnancy Complications
Criteria Computation Actual Score Justification

1. Nature of the Problem 1/3 x 1 1/3 Pregnancy is identified as a foreseeable crisis.

2. Modifiability of the Problem 2/2 x 2 2 It is easily modifiable because the resources in their
community are available and accessible for mothers like Mae.
The problem in the food supply of the family has been
addressed since they received support and have started a
small business to increase their source of income. Moreover,
their rental house offers enough space for Mae to have
enough rest in the course of her pregnancy.

3. Preventive/Potential 3/3x1 1 The possibility of Mae to prevent complications during

pregnancy is high since the family was able to rent a house to
accommodate their need for living space and they received
financial support to aid them in their daily needs, especially
food.

4. Salience of the Problem 1/2 x 1 1/2 The family has a low salience and does not fully recognize the
existence of the problem or of the foreseeable crisis as
evidenced by the lack of focus and attention to her pregnancy.
Too, since the gaps between the age of the children are not
idealistic, one can say that they do not fully comprehend the
gravity of the possible problems entailed with unplanned
pregnancies considering their living conditions.
