Philippines, after being sworn to in accordance with law, deposes and
states, that:

1. On April 22, 2016, I decided to buy a new motorcycle more

particularly described as follows:

2. To have more use of the said motorcycle in my business, sometime in

June 2016, I buy a side car to ABEL LIBAO;

3. I decided to register the said motorcycle with Land Transportation

Office with its side car;

4. However, the said side car was registered to its previous motorcycle
more particularly described as follows:

5. The said change of motorcycle is not intended for any illegal or

above-described motor vehicle on its Certificate of Registration.
