AFFIDAVIT OF UNDERTAKING

I, JED SALVIO PARACHA, of legal age, Filipino, single, with residence and postal
address at Purok 4, Poblacion A, Mlang North Cotabato after having been sworn to in
accordance with law, do hereby depose and state that:

1. That I am the same person JED SALVIO PARACHA who executed this affidavit;

2. That I am applying for the position Fire Officer 1 of the Bureau of Fire Protection
Region XII;

3. That I am a graduate of Bachelor of Arts Major in Public Administration and

Bachelor of Secondary education Major in Social Studies at Southern Baptist College
on the year 2011 and 2014, as well as Jurist Doctor (Bachelor of Laws) at Central
Philippine University last 2019;

4. That one of the requirements for the position Fire Officer 1 are the authenticated or
certified true copies of my diploma and/or Transcript of Records;

5. That I cannot comply such requirements because my Authenticated copies of my

Diploma and Transcript of Records are still at Central Philippine University;

6. That I shall comply with such requirement on or before the oath taking ceremony;

7. That I declare none further;

8. I am executing this Affidavit in order to attest the truthfulness of the foregoing

allegations and for whatever lawful intents it may serve best.
