VOLUNTARY COMMITMENT

AND ACCOUNTABILITY FORM

I, the undersigned, hereby commit and undertake to be bound by the following responsibilities:

1. Unless otherwise categorically stated in writing, it is understood that all information,

data, documents, and artifacts, or any matter seen, heard or encountered by the
undersigned during the Division Online SBM Validation will only be of sole and exclusive
limited use for the said purpose and only during the said Validation. Hence as a Member
of the School SBM Validating Team, all forms of downloading, printing, spreading, or
giving of comments, engaging in outside discussion beyond the given schedule, or any
form of disclosure directly or indirectly by act or negligence, shall be absolutely
prohibited. It is understood that said violation will result to disciplinary sanction pursued
under DepEd Order No. 49, Series of 2006, without prejudice to other sanctions
pursuant to available legal remedies such as a civil action for damages, and other
appropriate administrative or criminal action.

2. I freely, intelligently and voluntarily consent to be bound by the foregoing, with full
understanding of my responsibilities herein as it has been explained to me and to which
I confirm I accept. All other relevant laws, Civil Service Commission and Department of
Education issuances are also hereby adopted.
