Area/Concept: Patient Care Delivery System

1. Briefly discuss the scenario / events / reading.

Nursing profession brought about significant cornerstone when it comes to

utilization of both knowledge and skills that are essential in managing and integrating
best nursing practice towards the patient. As it continues to evolve in different
strategies, theories and even care model are being formulated to elevate the approach
of healthcare and provision of nursing practice when dealing to the client needs. In
relation to this, patient care delivery system were presented which aims to define the
way of work assignments, responsibility, and authority that are structured to accomplish
patient care. It also depicts which health care worker is going to perform what tasks,
who is responsible, and who has the authority to make decisions.
Moreover, from this concept different modalities of care or known as nursing care
delivery models are introduced to fully understand and equipped us with the idea on
how the health care team especially nurses functions and plan treatment for the client in
different areas of practice such as in community and even in the hospital institutions.
Models of nursing care delivery are as follows; case method, functional, primary
nursing, tea, nursing, modular nursing, nursing case management, and innovative or
contemporary method. Consequently, in assessing learning outcome about this topic or
concept we conducted a simple interview on some nurses for us to find and ask what
type of nursing care delivery model was used in their area of practice. Hence, through
this activity we are able to distinguish and identified specific models which hastens our
critical thinking and provide baseline information.

2. How do you feel about it? (You may include comments and conclusions)

This activity gave us a sense of productivity and interest as it logically explores

the different functions, responsibility and even collaborative work of health care team
as they exhibit and practice their profession in a certain settings or workplace.
Perhaps, from the information I gathered from one of the staff nurse I’ve interview
which is working abroad (Middle East) in a certain private institutions she asserts the
following idea:
- Nursing department is governed by Executive Director of Nursing. He provides
strategic directions to the department assisted by Directors of Nursing (DoN). Head
Nurses function as nurse managers of nursing units (ambulatory care and in
patient)

- HNs manage the day to day provision of nursing care services in his/her respective
unit. This includes staffing, availability of equipment and other resources so the unit

can function efficiently and effectively, guidance and monitoring the compliance to
highest standard of quality care as per JCI standards and in compliance with other
governmental agencies and accrediting body, conflict resolution among staff, staff well-
being, ensuring the unit participates in quality improvement projects etc.
- HN is assisted by Charge Nurse who provides guidance to Staff Nurses at bed side.
He/she is on duty 24/7 like Staff Nurses. SNs work at bedside and provides quality
patient care at bedside. SNs are empowered, they are leaders of unit-based
committees which was created to take the nursing practice to the next level, i.e. leading/
participating in quality improvement projects,, health teaching, etc

On the other hand, I was amazed by the another information I have gathered from our
community nurse who works for about 5 years now here in our rural health unit wherein
she claims that “mainly most common delivery model that we are utilizing in dealing
with our functions and task in a large population are team nursing, modular, and even
nursing case management.” Therefore, from this two perspectives of nurses working in
two different sides of health care setting we can say that delivery care models elevate
and expand the nursing practice into much accessible and optimal patient-centered
approach.

3. What have you learned from it?

As a student nurse, this task provides greater learning and understanding on how a
healthcare system functions and participate into deeper management of certain illness
and deficits. The aforementioned care delivery models above facilitates specific
strategies on how delegation and division of work runs into a certain group or health
care team. Furthermore, it also emphasizes the collaborative effort and defining work of
every nurses and other health care professionals. It also deals with the communication
process between each individual particularly in developing plan of care to patient. For
instance, direct client care to a group of client under the direction and coordination of
certain team leader allows task to be divided and full cooperation were ensure. Lastly,
additional concept were highlighted in this task were about application of all nursing
delivery care models even in two different perspectives and work settings.
