Doc Set 1: Reservation Requirements


To be submitted within 7 days

1. Reservation Agreement Form


2. Reservation Fee (Cash or Dated Check)
3. Proof of Billing
4. Proof of Income
5. 2 Government Valid IDs
6. Attorney In Fact (AIF) Form (if applicable)

UPON OFFICIAL SALE


1. Submit the required PDCs for DP & MF UPON Official Sale

Doc Set 2: Official Sale Requirements


(To be submitted within 15 days after reservation otherwise comm 2 will be on
hold)

A. For Employed
1. Proof of TIN
2. Bank Clearance (if with CMAP findings)
3. Marriage Contract / Birth Certificate & CENOMAR
4. 3 pcs - 1x1 photo
5. SPA (notarized / consularized)
If notarized – submit Philippine Arrival and Departure Stamp
6. Other Proof of Income not submitted upon Temporary Sale
(Payslip, ITR, COEC, Job Contract – OFW)

B. For Self-Employed
1. Proof of TIN
2. Bank Clearance (if with CMAP findings)
3. Marriage Contract / Birth Certificate & CENOMAR
4. 3 pcs - 1x1 photo
5. SPA (notarized / consularized)
If notarized – submit Philippine Arrival and Departure Stamp
6. Other Proof of Income not submitted upon Temporary Sale
(6 months Bank Statement, Financial Statement & Business
Registration)
