Critical analysis of radiofrequencyablation for trigeminal neuralgia:
Case series

Background: Trigeminal neuralgia is a condition in which severe
attacks of pain occur in one side of the face. The attacks are usually short
in duration, but are very severe. They can occur only occasionally,
or many times in a day. Radiofrequency ablation is an interventional
treatment which is performed using a special needle, can be used to
destroy the pain fibres in the nerve without affecting the touch fibres.
Objective: To evaluate the efficacy of radiofrequency ablation in
trigeminal neuralgia patients.
Patients and Methods/Material and Methods: This case series study
involved six patients that had been diagnosed as trigeminal neuralgia
clinically. Each patients were examined using ID Pain, Numeric Pain
Scale (NPS) before and after radiofrequency ablation.
Results: This case series study involved six patients that had been

Acute flaccid paralysis after asthma attack: A probable Hopkins

Background: Hopkins, in 1974, reported 10 children who subacutely
developed an asymmetric flaccid poliomyelitis-like paralysis occurring
less than 1 week after an acute asthmatic attack with poor recovery.

Objective: Report a probable case of this rare syndrome and the findings
diagnostic studies.
Patients and Methods/Material and Methods: A nine years old boy
was admitted in the emergency department of the Materno Infantil
Hospital at Brasilia/Brazil with an acute asthma attack characterized by

severe wheezing and dyspnea. The patient underwent corticosteroid
therapy intravenous (IV) and on the third day, he start to complain of
pain and weakness of the left leg. Physical examination revealed a grade
II proximal and grade III distal for the movements of the left leg and
diminished reflexes as well.
Results: CSF revealed elevated protein (144), normal cells count and
glycose. EMG showed normal sensory nerve conduction, reduction of
amplitude of CMAP of the left tibial e peroneal nerves and normal distal
motor and F waves latencies, as well as reduced recruitment of the motor
unit action potencials of the left anterior tibial and gastrocnemius
muscles. T2-weighted MRI of thoracic and lumbosacral spine revealed a
high-intensity area in the left ventral and dorsal horn at T10-T12 levels.
The patient were treated with 5 days of IV methylprednisolone and
human immunoglobulin showing no response to the treatment.
Conclusion: This case exemplifies a rare cause of a poliomyelitis-like
illness and that future research is needed to clarify its physiopathology
and the best treatment option.

Childhood epilepsy and its predictors

Background: Epilepsy is often left undiagnosed in Pakistan and it is a
leading cause of death.
Objective: The purpose of this study was to determine the prognosis
of seizures in epileptic children and identify its early predictors.

Patients and Methods/Material and Methods: A case control study
was conducted Sir Ganga Ram Hospital, Lahore. The duration of study
was one year. All children under the age of 1 month- 16 years having
idiopathic or cryptogenic epilepsy were included in the case control
study. Two groups cases and controls respectively. The children with
seizures even after the treatment of epilepsy were characterized as
(cases of) intractable epileptic seizures. Children who had no history
of seizures after adequate treatment and were on follow up were
labelled as controls. Adequate treatment includes at least three anti-
epileptic agents alone or in combination. Records of these patients
were reviewed to identify the variables.
Results: Of 652 epileptic children, 443 (68%) intractable and 208 (32%)
control epileptics were included in the study. Male gender (OR=3.92),
seizures onset in infancy (OR=5.27), ≥ 10 seizures before starting
treatment (OR=3.76), myoclonic seizures (OR=1.37), neonatal sei-
zures (OR=3.69), abnormal EEG (OR=7.28) and cryptogenic epilepsy
(OR=9.69) and head trauma (OR=4.07) were the factors associated
with intractable epilepsy.
Conclusion: Seizure onset between 5-7 years of age, idiopathic epilepsy,
and absence seizures were associated with favorable prognosis in
childhood epilepsy. Intractable childhood epilepsy is expected if certain
risk factors such as type, age of onset, gender and cause of epilepsy
are found. Early referral of such patients is recommended for optimal
management.

Background: Thalassemia is one of the most common inherited blood
disorders worldwide. Its complications including cognitive impairment
such as deficits in working memory. Working memory impairment is
associated with poor academic achievement which will influence quality
of life.
