Dugs that Affect Immune System

Case Scenario: Critical Thinking

Situation

Mr. Apidra is a 40-year-old male client who has been diagnosed with advanced acquired
immune deficiency syndrome (AIDS). His CD4 cell count is 145. His primary care provider
has Rx azidothymidine (AZT also called zidovudine) (Retrovir) 200 mg t.i.d. to be
administered concomitantly with zalcitabine (Hivid) 200 mg p.o.. Before starting the client on
combination therapy, the nurse continued to closely monitor his neurologic status and check
his complete blood count (CBC) and CD4 count

1. What is the normal range value of CD4 cell count and why is it important to monitor
CD4 cell count in client who are being treated for HIV infection?.

A. Normal cell value of CD4 cell count: 500-1,200 cells/mm3

B. Why:

B.1: If too many CD4 cells are lost, your immune system will have trouble fighting off
infections.

B.2: 250–500 cells per cubic millimeter. It means you have a weakened immune
system and may be infected with HIV.

B.3:The test can also check to see how well HIV medicines are working.

2. What is the significance of burning and tingling in the fingers and toes for clients
taking nucleoside reverse transcriptase inhibitors?

A. Burning and tingling in the fingers and toes are the potential side effect of certain
medications used to treat HIV/AIDS.

B. Nucleoside reverse transcriptase inhibitors (NRTIs), or “d-drugs”, are most frequently

associated with peripheral neuropathy.

C. This symptoms caused by these medications that can often be treated by reducing the
dosage or withdrawing the drug.

D. Side effects from taking NRTIs vary, depending on the individual.

3. Why is it important to closely monitor the complete blood count of clients taking
zidovudine (AZT)?

A. Because Zidovudine may decrease the number of certain cells in your blood, including
red and white blood cells

B. And this may result blood disorders such as anemia (a lower than normal number of red
blood cells)

C. Including bone marrow problems.

4. What drugs should the client avoid while taking zidovudine (AZT) because they can
cause bone marrow suppression?

A. dapsone (Avlosulfon)

B. ganciclovir (Cytovene)
C. ribavirin (used as treatment for hepatitis C)
D. valganciclovir (Valcyte)
E. valproic acid (Depakene, Divalproex, other brand names)

5. Drug classification of zalcitabine and its usual indication.

Drug Classification Usual Indication

It is in a class of drugs called reverse Zalcitabine is an oral medication that is

transcriptase inhibitors which also includes used for the treatment of infections with the
lamivudine (Epivir), zidovudine (Retrovir), human immunodeficiency virus (HIV).It is
didanosine (Videx), and stavudine (Zerit). used in combination with other medicines.
Zalcitabine in a nucleoside reverse
transcriptase inhibitor. It works by blocking
the growth of HIV.
