Dear Mr. Dela Cruz:

Greetings!

I am writing this letter to you to formally let you know that I am now reporting back to my duty.
My 2 days of absence has benefitted my physical health and helped me regain my strength to
maximize myself in rendering my duties to this office. I would also like to ask for the
forgiveness and understanding for not being able to formally ask for permission.

At the first day of my absence, I really suffered a headache that disabled from enduring the pain
and made me decide to take a break; even asked for a home-based medication and I was
informed to take another day of rest to not to push myself and get worse with my aches.

Today, being able to be present at my duty, I assure you that I will be working in my full
potential to fulfil the duties of the days that I am absent.

Thank you so much for the granted 2 days break and hoping to make more success with you in
this office.
