number __________________________________ do hereby allow my ward to attend the physical classes at Academy
premises as communicated through letter dated ______________commencing from 15th February, 2021 amid COVID 19.

I hereby undertake as under:

1. We understand that it shall be the responsibility of my ward to follow government and Academy’s guidelines and
will ensure that my ward follow the same.

2. We understand that it shall be the responsibility of my ward to maintain the complete social distance and hygiene
to ensure safety and health and will further ensure that my ward follow the same.
3. We understand that it shall be the responsibility of my ward to cover his/her face through face mask and use hand
sanitizer for taking the appropriate precaution against the COVID 19 virus and will ensure that my ward follow the same.

4. We understand that the Academy is following the SOPs for taking precautions against the spread of COVID.
Academy shall not be responsible in any way for any COVID related health concerns, that may arise with my ward.
