Discussion:
Dr Smith works in general paediatrics with a special interest in haematology. She has documented 75 hours
of CPD activity related to these areas, including current life support training. She gives 7 lectures on the
undergraduate paediatric course and presented 4 posters and 3 oral presentations at national or international
meetings in 2010. She has completed her MD thesis on management of thrombocytopenia in the past year.
Specific skills that were appreciated in feedback on lectures include involving students and providing
references for further study. She identified an area for development around the projection of her voice.
Good quality detailed inpatient and outpatient records and examples of referrals to other colleagues included.
Dr Smith asked a colleague to review her records and has reflected on the results. No specific change in
practice was required.
Dr Smith feels that she has settled well into the department and the department works well as a team. We
discussed her induction into the department. She told me that the team works together well and we discussed
how she could provide supporting information to demonstrate this for a future appraisal. See Action Domain
3. She has been able to develop her particular interest and this is well demonstrated by her establishing a
Scotland-wide paediatric cardiology video conference group.
Discussion:
Dr Smith has completed CPD in risk management. She has provided evidence for safe supervision of
students and junior staff, and how she passes care of children to colleagues at end of shift. She has provided
letters from trainees expressing their thanks for the training and supervision that she has given.

She stated that her own health is good and she has signed the health declaration.
Issues: No concerns
Action:
She has been able to build into her practice and CPD considerable action to promote patient safety and will
continue with this. In particular she will seek opportunities to use and demonstrate her risk management skills
in the clinical service. In the absence of issues, I suggest that no specific actions are being recorded here.
Discussion:
Dr Smith told me that she had not received any complaints this year. We discussed how she ensures good
communication with colleagues in department and laboratory including regular meetings. She has provided
examples of information given to parents on specific conditions which were written in plain English and are
being translated into other languages.
Multi-source feedback and patient feedback were not done this year, but will be required for revalidation next
year.
Research consent compliance is in place. Dr Smith provided examples of ethical approval and consent
forms.
Issues: None
Action: Dr Smith aims to organise MSF and patient feedback before the next appraisal.

Discussion:
We discussed confidentiality. Dr Smith stated that she is aware of GMC guidance and she provided letters
and emails that showed she is aware of and abides by the rules of confidentiality and e-mail activities.

Probity form signed with no issues identified. Dr Smith does not undertake private practice.
Fees for lectures get paid into departmental funds, and spending decisions are all approved by the
department consultants committee.
She has completed equality and diversity training and doctors in difficulty training.
Issues: Modern communication systems such as email create new opportunities and expectations for rapid
communication with parents, but also carry risks to confidentiality. This is particularly complex when two
parents are no longer together.
Action: To raise the question of email use at the departmental consultants meeting, or possibly elevate to the
college, and attempt to reach some consensus guidance.
