Information for Parents - School Year 2021-22

Register for home schooling:
If you plan to continue Fill out the 2021-22 Letter of Intent (LOI) survey for all returning

Obtain forms, templates, or get more information:
Download a copy of Part Go to our webpage and scroll to Documents. All forms are fillable PDF,
100.10, Q&A and forms including the Individualized Home Instruction Plan (IHIP), quarterly
report, and Annual Assessment forms.
Submit IHIP, quarterly Email documents to homeschool@schools.nyc.gov. IHIP submissions
reports and assessments will receive a Letter of Compliance when approved.
Arrange for a call back for Email homeschool@schools.nyc.gov; include parent and child name,
individual questions telephone number, and best time to call. Please note calls may
originate from a blocked or unknown number.
Obtain curriculum The NYS Education Department sets student learning standards and
resources does not require the use of a specific curriculum. See Part 100.10 (e)
for required subjects in grades 1-6, 7-8, and 9-12. There are no State
required courses for students in kindergarten or pre-K grades.
Obtain a MetroCard:

Child Performer Permit Child Performer Permits are available on the NYS Department of Labor
LS 561 web page. Send the completed form to homeschool@schools.nyc.gov.

Social Security Advance A Form SSA 1372-BK is required in order for a student to continue
Notice of Termination of benefits after they turn 18. Complete pages 1 and 2 and email the form

Apply to public elementary, middle, or high school:
Access mySchools Families can use MySchools to apply to public schools from 3-K to high
school, as well as apply to sit for the Specialized High Schools
Admissions Test (SHSAT). Email homeschool@schools.nyc.gov to
obtain an Account Creation Code.
Return to public school:
Return to a public school You may return your child(ren) to the New York City public school
system at any time. Reach out to a Family Welcome Center for
assistance and information on placement options.
Request discharge from NYC for a:
Student who is beyond Email homeschool@schools.nyc.gov; include parent and student name
compulsory age or has and either confirmation that the student has completed home schooling
passed the TASC exam or the TASC exam results.
Permanent move outside Email homeschool@schools.nyc.gov and provide your street address,
