Affidavit cum Undertaking

_________________________________________, do hereby solemnly
affirm an oath as under:-

I say that, I have purchased a new vehicle of make ___________

and model ________________ with Engine No._________________ and
Chassis No____________________ to register as taxi in the office of
RTO___________________________________.

1. I say that I have attached a Grant Letter issued by State

Transport Authority, Panaji for the said vehicle to be registered
as Taxi/Two Wheeler (Yellow Black)/Rent a bike / Rent a Car.

2. I say that I have attached my proof of residence, Sale letter,

Invoice and vehicle insurance copy covered for the period of
one year issued by ______________________ Insurance
Company.

3. I say that now I understood from the RTO Authority that as

per the Judgment of the Honorable Supreme Court and the
notification from Ministry of Road Transport & Highways the
new vehicle to be registered under transport category has to be
covered with the insurance for the period of three /five
years.

4. I say that no insurance company issues the insurance for a

period of three /five years in respect of Transport category
vehicles.

5. I say that, before the expiry of the insurance cover for this
one year I shall renew the insurance cover for next two years
or till such time the Insurance Company come with the
Insurance scheme of three/five years whichever is earlier,
failing which the registration of my vehicle would be liable to
be terminated.

6. I say that whatever stated above in Para No 1 to 5 are true to

the best of my knowledge and belief.
