FOR ACADEMIC YEAR (AY) 2020-2021 DATE 12 JULY 2020 To assist the Commission in making recommendations to the Inter-Agency Task Force for the Management of Emerging Infectious Diseases, higher education institutions (HEIs) are enjoined to provide information on their preparations and readiness in the implementation of flexible learning for Academic Year (AY) 2020-2021. HEIs can share information through this _link tiny. co/HE|ReadinessSurvey. HEIs without internet connectivity, may accomplish paper copies of the questionnaire (see attached) which will be encoded online by the CHED Regional Office (CHEDRO) upon receipt thereof. HEls far from the regional offices, may share information through SMS or messaging apps to the CHEDROs upon verification of the identity of the authorized HEI representative. We aim to cover all HEls to ensure that all voices are heard in this time of uncertainty. The online collection system will be closed by 11:59 PM on 20 July 2020. Responses may be edited or added any time before the deadline by saving and revisiting the “Edit your response" link that will be shown after you submitted the accomplished Google form. Kindly ensure that only authorized persons are able to access the link to avoid unauthorized editing of your responses. Altematively, HEIs may also use the attached questionnaire to gather data from other offices in the HEI before submitting online. CHEDROs are instructed to widely disseminate this memorandum and its attachments through various media of communication (email, text, fax, and social media) and to ensure the timely submission of information from all HEIs especially those with no connectivity to provide a clearer picture of the situation in your respective regions. ‘Should HEIs have further inquiries, our respective CHEDROs can be reached through communication channels indicated in the attached directory.
