INTRODUCTION

Family is being defined as the basic unit of the society. It is


where everything starts. Life started with them and be with them
through all the way as you grow and of course has a big impact in all
aspects of your life like mental, physical, emotional and even health.
Health is a very crucial part in the family as it will define all the
actions or movements in the family. Being unhealthy will affect the
entire well-being of the family. Identifying and solving them can be
done by the nurses. Nurses’ duty doesn’t just stop inside the hospital
but rather extended in the community specially in the family.
Conducting assessments and data analysis will make a bridge to a new
knowledge and leading the family to a more serious complication. Data
gathering will give you a baseline for the diagnosis and interventions
that will done for the family. So, by this, conducting a home visit is
the key for making these things successful. By conducting a home
visit, we can easily assess different health risk and factors to the
family like environment, economic or even health history.
As a Level II student nurse, I was assigned to conduct a home
visitation on a family as a part of the clinical rotation in the
Community Health Nurse. So, I choose Family P as the subject of my
Individual case study. I interview them in different aspects in their
life that will make lighten different factors or risk to the health of
the family such us their economic status, cultural background,
environmental factors and many more. By this, we can identify any risk
in the family that may affect the health of the family.
So, by the end of the home visit, it was expected by the nurse to
have a diagnosis and intervention that is implemented in the family.
It was expected too that the family will cooperate with the nurse and
will actively participate in the home visitation and in the
intervention to be guide and implement by the nurse. It was also
expected that these practices will not just stop during the visitation
but will continuously do even after the visitation so that they will
be free or reduced the risk in the health status of the family.
