JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, ISRAEL M. PILAPIL and HELEN C. PITAO, of legal age, Filipinos and residents of Los
Angeles, Butuan City, UNDER OATH, hereby state :

1. We personally the late BARTOLOME MIOLE MONTEDERAMOS, who died on

August 27 2020 and who during his lifetime, was married LILIA O.
MONTEDERAMOS and resided in P-17, Los Angeles, Butuan City;

2. We categorically state that –

a. The person named as BARTOLOME MIOLE MONTEDERAMOS in a Death

Certificate no. 2020-1817 with date of Birth November 4, 1949;

b. The SSS member named BARTOLOME MIOLE MONTE DE RAMOS with date of
Birth 01-06-1950;

c. The person named as BARTOLOME RAMOS in a Birth Certificate no. 105-s/50

with date of Birth January 21,1950;

d. The person named BARTOLOME MONTEDERAMOS in that marriage contract

with the former LILIA OLAR signed by Judge Remedios Alvizo, Municipal Judge
of Bislig, Surigao del Sur;

e. The person appearing in those SSS membership cards (one bearing SS no.
08154-988) as BARTOLOME MONTE DE RAMOS;

all refer to one and the same person - late BARTOLOME MIOLE
MONTEDERAMOS, who died on August 27 2020 and who during his
lifetime, was married LILIA O. MONTEDERAMOS and resided in P-17, Los
Angeles, Butuan City;

3. Said person’s true and correct name is BARTOLOME MIOLE MONTEDERAMOS,

as we personally know his parents- TEODORO MONTEDERAMOS and the former
MARIA MIOLE . However, we cannot ascertain his correct date of Birth .

4. Said errors are mere typographical brought about by the fact that some people
do not consider the prefix “de” as part of the surname.

5. We know these facts being long time neighbors of said persons and his family.

6. We categorically state herein that no other person will claim as

BARTOLOME MONTE DE RAMOS, who is an SSS member (with SS no. 08154-
988) other than late BARTOLOME MIOLE MONTEDERAMOS, who died on
August 27 2020 and who during his lifetime, was married LILIA O.
MONTEDERAMOS and resided in P-17, Los Angeles, Butuan City;

7. We execute this affidavit to attest to the truth of the foregoing.
